Your Portal login is the same as your My BackPack login. If you do not recall your My Backpack user name or password, go to the My BackPack login page and click "I Forgot My User Name/Password" (fastest method for help). Once you have retrieved your user name and password, return to the School Portal and enter that information to log in. Ursuline Academy uses an online database called Magnus Health SMR (Student Medical Record). This secure web-based system houses school-related forms, including:
Immunization Form needed for new families to complete enrollment
Athletic forms required for all team sports participation (including cheer and Rangerettes)
Over-the-counter medication forms
Consent to treat, and other health forms
