A VOE is required when a student has an appointment with the Texas Department of Public Safety (DPS) to obtain a driver’s license or learner’s permit. Please note that a VOE is valid for 30 days only.

Students who have a DPS appointment scheduled and need a VOE should follow these steps:

1. Email Ms. Ayotte with VOE Request in the subject line.
2. In the body of the email, type the student’s full name exactly as it should appear on the form.
3. Mrs. Ayotte will send an email when the VOE is ready for pickup.
4. Students may pick up the form from the pink folder on Mrs. Ayotte’s desk. Forms are filed alphabetically by last name.

Please note:

• VOE requests must come directly from the student.
• Requests submitted through Teams chat will not be accepted.
• Parents should not request VOEs on behalf of students.
• Students should not wait until arriving at the DPS office to request a VOE, as the form cannot be emailed at that time.
